X-ray frequency should be tailored - not "the same every visit." Here is what the ADA and ALARA guidelines actually recommend, by age and risk.
The ADA framework
The American Dental Association recommends X-ray frequency based on risk-adjusted clinical judgment, not a fixed schedule. Key considerations: cavity risk (history of cavities in past 24 months), periodontal status, age, current symptoms, and time since last X-rays. The ALARA principle (As Low As Reasonably Achievable) means we don't over-image, but we don't skip clinically necessary imaging either.
Adult schedules - typical
Low-risk (no recent cavities, healthy gums): Bitewings every 18-24 months; full-mouth or panoramic every 3-5 years.
Moderate-risk (occasional cavity, mild gum issues): Bitewings annually; full-mouth or panoramic every 3 years.
High-risk (active cavities, smokers, periodontal disease, diabetes): Bitewings every 6-12 months; full-mouth or panoramic every 2-3 years.
New patients: Comprehensive imaging at the first visit (usually a panoramic plus 4 bitewings) to establish baseline.
Pediatric schedules
Primary dentition (under 6): X-rays only when clinical concern exists. First X-rays often around age 4-6 once back teeth touch.
Mixed dentition (6-12): Bitewings every 12-24 months depending on cavity risk; panoramic around age 8-10 to evaluate developing adult teeth.
Adolescents (12-18): Bitewings every 18-36 months for low-risk; annually for higher-risk. Panoramic before orthodontic treatment and before wisdom tooth evaluation.
Special situations
Pregnancy: defer non-emergency X-rays to after delivery. Recent extraction or implant: follow-up periapical at 6 months and 1 year. Active orthodontic treatment: periodic panoramic during long cases. TMJ symptoms: sometimes CBCT. Implant planning: always CBCT before placement.
Bringing in prior X-rays
If you have recent X-rays from another office, request them sent to us before your visit. Bitewings within 12 months are usually still useful and don't need to be retaken. Panoramic within 3-5 years may not need retaking if no significant changes. Bringing prior records reduces both your radiation exposure and your bill.
Frequently asked questions
- My dentist wants X-rays at every cleaning. Is that necessary?
- Bitewings every 6 months may be appropriate for high-risk patients but is not standard for most adults. Ask about the rationale and your specific risk factors.
- Can I skip X-rays to save money?
- Skipping clinically necessary X-rays risks missing problems that become much more expensive to fix. Some can be stretched if your risk is low - discuss with the dentist.
- Will my insurance cover dental X-rays?
- Most plans cover bitewings yearly and panoramic every 3-5 years at the preventive tier (100%). Additional X-rays may be covered as basic restorative when clinically indicated.
- Can I refuse X-rays?
- You can - but if X-rays are clinically indicated and you decline, the dentist may decline to provide treatment that depends on them. We document refusals and explain consequences.
