Composite resin (tooth-colored fillings)

Composite is the workhorse of modern dentistry. It bonds chemically to enamel and dentin, comes in dozens of shades, and can be sculpted directly in the mouth with no lab step. Lifespan: 7-10 years on small fillings, longer with conservative occlusion. Best for: small-to-moderate fillings, chairside veneers, and minor cosmetic bonding. Limitations: less wear-resistant than ceramic, can pick up stain over time, and slightly more technique-sensitive to place.

Porcelain & lithium disilicate (e.max) - front-tooth aesthetics

Lithium disilicate (brand name e.max) is the gold standard for front-tooth crowns and veneers when aesthetics matter most. It transmits light like natural enamel, comes in fine shade gradations, and is strong enough for single-tooth front and premolar crowns. Lifespan: 15+ years with care. Best for: anterior crowns, veneers, inlays/onlays. Limitations: less ideal than zirconia for heavy-load molars in patients who clench.

Zirconia - strength for molars and bridges

Zirconia is a ceramic that rivals metal for strength while staying tooth-colored. Modern multi-layer zirconia is also reasonably aesthetic. Lifespan: 15+ years; some studies project 20+. Best for: posterior crowns, longer-span bridges, implant restorations, patients who grind. Limitations: high opacity makes pure zirconia less aesthetic than e.max for very visible front teeth (mitigated by layered designs).

Porcelain-fused-to-metal (PFM) - the legacy hybrid

PFM crowns have a metal core capped with tooth-colored porcelain. They were the workhorse from the 1970s through the 2000s and still have a role for some long-span bridges. Lifespan: 10-15 years typical. Limitations: a thin "metal margin" can show as a dark line at the gum line if recession occurs, and the metal core makes them less aesthetic for front teeth. Most offices have largely shifted to all-ceramic alternatives.

Gold and gold-alloy restorations - longest lifespan

Gold inlays, onlays, and full-cast crowns can last 30+ years with virtually zero failure. Gold flexes with enamel under bite pressure, sealing the margin against bacteria far more reliably than any ceramic. The only drawback is appearance, which is why gold is now usually reserved for back molars in patients who prioritize longevity over color match. Limitations: visible color, higher up-front cost (driven by the metal price), generally not insurance-friendly.

How we choose for each case

Material choice depends on the tooth's position, your bite force, what is left of the tooth, and your aesthetic priorities. For a small filling on a back molar, composite. For a large molar restoration where the cusps are gone, an onlay in zirconia or e.max. For a single-tooth front crown, e.max. For a multi-unit posterior bridge, zirconia. For an implant crown on a back molar in a heavy grinder, monolithic zirconia. We discuss the trade-offs at every visit so the decision is yours, not assumed.

Frequently asked questions

Are silver (amalgam) fillings still used?
They are still legal and effective, but our office has not placed amalgam in many years. Composite bonds to the tooth, looks natural, and removes far less healthy enamel during preparation.
Will my zirconia or e.max crown match my natural teeth?
Yes - both materials come in extensive shade libraries. We use a digital shade-matching tool and can also send to the lab a custom shade map to mimic translucency, mamelons, and surface texture for the best match.
How long should a crown actually last?
A modern all-ceramic crown should last 15-20+ years with good home care, regular cleanings, and a night guard if you grind. The most common failure mode is decay at the margin, not fracture of the crown itself.
Are there any safety concerns with any of these materials?
No. Composite, ceramic, zirconia, and gold are all biocompatible and FDA-cleared. Concerns about BPA in composites have been addressed by modern BPA-free formulations. We are happy to share material safety data sheets on request.
